POLICIES & CONFIDENTIALITY
Counseling & Psychological Services (CAPS) documents are separate from a student’s academic/educational records. ONLY CAPS staff has access to them. CAPS will maintain and retain such records in a confidential manner, except in specific situations where disclosure of information is ethically or legally required (see Services, Policies, & Confidentiality below for additional information). The CAPS Office will retain a student’s record(s) in accordance with legal and ethical standards. Aside from the typical limits of confidentiality, a student’s clinical records can only be disclosed if an “Authorization to Release Information” form (available at the CAPS Office and/or through the CAPS Therapy Portal) is completed by the student.
Students who have scheduled an initial appointment (in person or Teletherapy/Video) may be granted access to the CAPS Therapy Portal, to complete the required intake paperwork. Students are asked to log into their Therapy Portal account and complete the aforementioned paperwork within 24 hours of their initial appointment. Students who do not complete the required paperwork in the above timeframe may be notified that their appointment has been rescheduled for the following week in the same timeslot. If the paperwork remains incomplete prior to the rescheduled appointment, the student’s appointment may be canceled. That student may be required to contact the CAPS for another appointment.
