POLICIES & CONFIDENTIALITY
Counseling & Psychological Services (CAPS) documents are separate from a student’s academic/educational records. ONLY CAPS staff has access to them. CAPS will maintain and retain such records in a confidential manner, except in specific situations where disclosure of information is ethically or legally required (see Services, Policies, & Confidentiality below for additional information). The CAPS Office will retain a student’s record(s) in accordance with legal and ethical standards. Aside from the typical limits of confidentiality, a student’s clinical records can only be disclosed if an “Authorization to Release Information” form (available at the CAPS Office) is completed by the student.
Students who have scheduled an initial appointment will be asked to complete intake paperwork through Therapy Notes. Students are asked to log into Therapy Notes and complete the aforementioned paperwork before their first appointment. Students who do not complete the required paperwork in the above timeframe may be notified that their appointment has been rescheduled. If the paperwork remains incomplete prior to the rescheduled appointment, the student’s appointment may be canceled.
